At Calibrate, we take your privacy seriously. Our Privacy Policy is designed to ensure that we are fully HIPAA Compliant at all times. This Privacy Policy explains how we access, use and share your personal data. By accessing or using our Services in any manner, you acknowledge that you accept the practices and policies outlined below, and you consent to our collection, use and sharing of your information as described in this Privacy Policy.
Your use of Calibrate's Services is subject at all times to our Terms of Use, which incorporates this Privacy Policy. Any terms we use in this Policy without defining them have the definitions assigned in the Terms of Use.
What this Privacy Policy Covers
This Privacy Policy covers how Calibrate Health, Inc. and its affiliated entities (collectively “Calibrate”) may access, use and share your Personal Data when you access or use our Services. “Personal Data” means any information that identifies or relates to a particular individual and also includes information referred to as “personally identifiable information” or “personal information” under applicable data privacy laws, rules or regulations.
This Privacy Policy does not cover the practices of third parties and/or companies that Calibrate doesn’t own or control or people we don’t manage. This Privacy Policy also does not cover Protected Health Information (as defined by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and HIPAA Privacy Rule) collected by the Practice (as defined herein) and providers with whom Calibrate contracts to facilitate medical visits through the Calibrate platform. Calibrate currently contracts with these medical groups (the “Practice”), however, the medical groups we contract with may change from time to time. The Calibrate Notice of Privacy Practices describes how Calibrate and the Practice may use and disclose your Protected Health Information. If you do not agree to those terms, which are available here, you should not access or use Calibrate services.
